I acknowledge that therapy costs are considered out-of-network services. Payment will be due in full at the time of service, however I will be provided with a superbill that can be submitted to my insurance company for potential reimbursement. I understand that while Pivot Point will provide these documents to me, it will my responsibility to file these with my insurance company if I would like to receive reimbursement for out-of-network provider services.
